How they work, when they help, and what they don’t change

PDE5 medications — including sildenafil (Viagra), tadalafil (Cialis), and vardenafil (Levitra) — are among the most commonly prescribed treatments for erection difficulties.

They’re often compared in simple ways. One lasts longer. One works faster. One is better for spontaneity.

That framing isn’t wrong. It’s just incomplete.

These medications don’t create erections. They improve blood flow responsiveness.

During arousal, the body releases nitric oxide in erectile tissue, allowing blood vessels to relax and fill. An enzyme called phosphodiesterase type 5 (PDE5) helps regulate that process. Sildenafil, tadalafil, and vardenafil partially block that enzyme, allowing the blood flow signal to last longer.

They reduce friction in one part of the system. The rest still has to show up.

If blood flow is the limiting factor, they can help meaningfully. If something else is limiting, results may feel partial.

How PDE5s Are Typically Used

People generally use PDE5 medications in three ways.

Daily low-dose tadalafil creates steady background support. It doesn’t usually make erections dramatically stronger — it makes them more stable and less easily disrupted. The tradeoff is continuous exposure and ongoing side-effect potential.

What They Change — and What They Don’t

Practical Considerations

PDE5 medications are prescription drugs and are generally well tolerated. Common side effects include headache, flushing, nasal congestion, indigestion, and back pain (more common with tadalafil). Less common but important considerations include vision changes, low blood pressure, and dangerous interactions with nitrates.
